Lasting Power of Attorney (LPA)

The Lasting Power of Attorney (LPA) is a legal document which allows you (the donor) to appoint a trusted person (the donee) to make decisions about your personal welfare, properties and affairs on your behalf when you lose mental capacity. This document needs to be signed by a Certificate Issuer, whose role is to certify that you, the donor, understands the implications of your decision to make an LPA.

Why do I need an LPA?

  • To protect your own interests and welfare should you lose the capacity to make decisions for yourself in the future

  • To ensure that you have the autonomy and choice of appointing someone you trust to make decisions in your best interests

  • To save your loved ones from the trouble of having to apply for Deputyship in the event that you lose mental capacity without an LPA in place (this takes longer and costs more)

Who can make an LPA?

Anyone who is at least 21 years old and has the mental capacity to make an LPA can do so. If you wish to apply for an LPA due to property and affairs matters, you must not be an undischarged bankrupt.

There are two LPA forms - LPA Form 1 and LPA Form 2.

  • LPA Form 1 is a standard version that allows you to grant your donee general powers with basic restrictions and is the most widely used form.

  • LPA Form 2 is for donors who want to customise and specify what powers they grant to their donees. If you wish to use LPA Form 2, you will have to engage a lawyer to draft the Annex to Part 3 of the form.

How do I make an LPA?

To make an LPA using Form 1, these are the steps:

  1. Download and complete the LPA Form 1.

  2. Our Speedoc doctor will come to your home to fill out Section 8 of the LPA Form 1.

  3. Submit the form and a photocopy of both the donor and donee’s NRIC/FIN/Passport (for foreigners) to the Office of the Public Guardian.

  2. Photocopy of both the donor and donee’s NRIC, or FIN/Passport for foreigners

How much will the certification cost?

Application fees for LPA 1 are waived for all Singapore citizens until March 2021! This is the best time to plan ahead and set up your LPA. At Speedoc, we charge $60 (excl. GST) for our doctors to witness and certify your LPA. This is on top of the house call fees of $200 per person (excl. GST).
