Advance Medical Directive (AMD)
The average life expectancy of Singaporean citizens has been steadily increasing annually, from 81.65 years in 2010 to 83.66 in 2020. This is thanks to the development of modern medicine and medical technology which have improved patient outcomes and made it possible to prolong life.
However, as a byproduct of increased lifespans, Singapore has also seen a rise in end-of-life illnesses such as dementia, Alzheimer’s disease, stroke and more, which result in decreased mental capacity and decision-making ability. As a result, legal documents such as Advance Medical Directives have been gaining popularity for people to convey their intentions before they reach that point in life.
What is an AMD?
An Advance Medical Directive (AMD) is a legal document that you can sign when you have the mental capacity to communicate to your healthcare providers that you do not wish to receive any extraordinary life-sustaining treatment, such as ventilators or life support machines, to prolong your life if you become terminally ill and unconscious.
Who can apply for an AMD?
Anyone who is 21 years old and above and of sound mind can make an AMD. Making an AMD is an entirely voluntary decision.
How can I apply for an AMD?
Complete the AMD form and print it out or obtain a physical copy of the AMD form from a hospital or polyclinic.
Consult a Speedoc doctor, who will have the responsibility to ensure that you are making the AMD on your own will, you are mentally sound, and that you understand the nature and implications of making an AMD.
Sign your completed AMD form in front of the doctor. The other witness can be a nurse if you prefer for this to be a personal decision. If this witness is your relative, they must not stand to gain anything from your demise.
Submit the completed and signed form to the Registrar of AMD by mail or in-person to: Registrar of Advance Medical Directives at Ministry of Health, Singapore, College of Medicine Building, 16 College Road, Singapore 169854
The Registrar of Advance Medical Directives will send you an acknowledgement when your AMD has been registered.

How much does it cost to make an AMD?
The cost varies depending on the price of the doctor consultation where the doctor is required to explain the implications of the AMD. At Speedoc, we charge $50 (excluding GST). The AMD form can be printed out for free here.
What if my family disapproves?
The final decision is yours entirely and the doctor will have to respect your AMD. However, it is encouraged that you discuss with your family regarding your wishes so that they would support and understand your rationale.
What if I change my mind after making the AMD?
You can revoke your AMD whenever you wish by completing a revocation form, AMD Form 3, in the presence of a witness.
